Fees & Arrangements Insurance Below is a list of Employee Assistance Programs, insurance companies or insurance company underwriters that I am contracted with for in network and out of network benefits. You can click on one of these links and it will take you directly to your insurance company's web site. I advise that you contact your insurance company to be certain that they will cover mental health services provided by me prior to attending your first session.

If you do not see your insurance company's name here it does not necessarily mean that I am not covered. Sometimes your benefits are managed by one of these companies. Follow these steps: Contact your insurance company prior to scheduling an appointment and ask them these questions:

Sliding Fee Scale If you do not have insurance please contact me to discuss my fee structure. If you do not have insurance or if your insurance coverage does not pay for mental health benefits you can pay out of pocket according to the sliding fee scale. This is a fee arranged after considering your household income to determine your fee. If you have insurance you must either use your insurance or pay full fee out of pocket. Your first session: What you can expect: Intake Session: The first and second sessions are an opportunity for us to get some paperwork out of the way (bring your insurance card) and an opportunity, for us to realize what can be expected from the work we will be doing together. I will also complete a Psychosocial assessment during these two sessions. You can help by printing the Personal Info Form (In Microsoft WORD) and complete it before our first session. All of this effort will make our work together much more productive. I look forward to working with you. |
