If you do not see your insurance company's name here it does not necessarily mean that I am not covered. Sometimes your benefits are managed by one of these companies, similar to how CDPHC is managed by United Behavioral Health. Follow these steps: Contact your insurance company prior to scheduling an appointment and ask them these questions:

How many outpatient sessions do I have (or...do I have left if you have already used some) for this calendar year? How much is my co-pay for each session? Will you give an authorization number? Sliding Fee Scale If you do not have insurance please contact me to discuss my fee structure. you can pay out of pocket according to the sliding fee scale. This is a fee arranged by considering your household income to determine your fee. What you can expect: Intake Session: The first and second sessions are an opportunity for us to get some paperwork out of the way (bring your insurance card) and an opportunity, for us to realize what can be expected from the work we will be doing together. I provide a full assessment, usually during the second session. Once the assessments are completed we will begin with regular 45 - 50 minute sessions. At the time of service, you will be expected to pay either, the payment rate scheduled through self pay or your co pay as determined by your insurance company. Once you have determined the method of payment, print off the Personal Info Form (In Microsoft WORD) and complete it before our first session. All of this effort will make our work together much more productive. I look forward to working with you. |
