Group Forms & Scheduling

You must attend an intake/screening session prior to getting assigned to group therapy.  Call your insurance company for an authorization number.  To speed up the process of registering for a group, print and complete the Registration Form then send it to me at the address on the HOME page or call to schedule an INTAKE session.  If you send the registration form, I will call you with an INTAKE appointment.  See Insurance & Fees page for more information.
